Greetings!
Thank you for your request dated Jan 04, 2021 10:59:56 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of your DOLE-AKAP Application.
OWWA Verification System shows that you applied online on April 24, 2020 was evaluated by OWWA-NCR as APPROVED on 12/20/2020. Your documents were forwarded to DOLE Region NCR for the release of financial benefit.
With respect to change of claimant, you or your wife must go directly to the DOLE Regional Director for your concern and know the procedure and documentary requirements to be submitted.
If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Administrator Hans Leo J. Cacdac at email: [email protected]. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter.
