Please submit alongside your university ID, an endorsement letter from your academic adviser, a copy of your resume/biodata, an NBI/PNP clearance, and a fully accomplished AFP FOI Request Form found in the Annex F of the AFP FOI Manual that you can download here:

Your right to request a review
