Hello po sir/mam, aalamin ko lang po kung ano po itsura ng award of "PA, PN/PN(M), PAF, and AFP Reservist Officer/EP of the Year" Medal and Ribbon? 

Existing pa po ba siya at pwede ma-award sa mga best reservist napili for the year? 

Kasi hindi po alam ng mga pagawaan ng awards/medals kung ano ba dapat itsura nitong award na to lalo na pag magpagawa sana ng ribbon ang mga reservist recipients netong award na to.

We regret to inform you that, per Section 5 of the AFP FOI Manual, in order to process your request, we will need you to submit the following:

1. A fully accomplished AFP FOI Request Form (Annex F) with reasonable description of the information being asked
2. Two valid government-issued IDs;
3. Clearances from the NBI, PNP, and Barangay; and
4. An endorsement letter signed by your research adviser that also details the scope and purpose of your study.

Alternatively, if you're from the AFP, please go through  the proper channels. You may request the same from the Office of the Deputy Chief of Staff for Operations, OJ3.
