This request is primarily for academic purposes and the requestor seeks a copy of a psychological report from an old and denied child adoption case petitioned within Tarlac province between 2015-2021. It is initiated to fulfill a case requirement in the course subject "Introduction to Forensic Psychology", as a graduate student of the PUP-Graduate school under Psychology program.

You asked for a copy of a psychological report from an old and denied child adoption case petitioned within Tarlac province between 2015-2021.

Response to your request

Unfortunately, the information you are requesting falls under the list of Exceptions to the Right
of Access to Information enumerated in the Inventory of Exceptions to Right to Access to Information issued by the Office of the President (attached as ANNEX B to the FOI Manual of the DSWD). Specifically, it falls under:

a. Information of a personal nature where disclosure would constitute a clearly unwarranted invasion of personal privacy, personal information or records, including sensitive personal information, birth records, school records, or medical or health records;
