May I please ask your assistance in confirming if my father is still listed in the 4P's program of our government he wasn't able to get his new card with micro chip in Manila since we have moved in Tarlac? Please see details below:

Details: Name - Ronie Sican Siena and Address - 205 E. Rodriguez Sr. Ave, Quezon City.

Response to your request

Unfortunately, the information you are requesting falls under the list of Exceptions to the Right of Access to Information enumerated in the Inventory of Exceptions to Right to Access to Information issued by the Office of the President (attached as ANNEX B to the FOI Manual of the DSWD). Specifically, it falls under:

a.	Information of a personal nature where disclosure would constitute a clearly unwarranted invasion of personal privacy, personal information or records, including sensitive personal information, birth records, school records, or medical or health records;

Your right to request a review
