Hi DSDW, I would like to ask some special request to your office if posible i can be a beneficiary of SAP in its 2nd wave. My name was never listed before as our brgy. Officials didnt include me. I am just staying in my sisters house and have no job for now. My source of income before was selling a siomai and other streets foods and because of pandemic i dont have my source of income anymore. Im hoping your office will honored my request as i know in this coming 2nd wave of sap my name will not be on the list. Im Basilio Sultan a a registered voter of Brgy. Diezmo Cabuyao Laguna. 5 June 2020 Dear Mr. Basilio Sultan, Greetings! Thank you for your request dated 20 May 2020 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You requested the following: (1) You would like to be, if possible, included as a beneficiary of SAP in its 2nd wave because you were not listed by your barangay and you are just staying in your sister’s house and you were a street vendor before. Unfortunately, you don’t have a source of income anymore because of the pandemic. You would like to have a barcode and apply it on SAP DSWD app. Response to your request Unfortunately, this is not a valid Freedom of Information (FOI) Request. Thus, we are constrained to deny your request. However, you may contact the DSWD Social Amelioration Program Hotline Numbers at telephone number 16545 or by cellular phones through calling 09474822864 (Smart), 09162471194 (Globe) or 09329333251 (Sun) or texting 09189122813 or by e-mail at [email protected] to inquire about your concerns and/or report irregularities about the Social Amelioration Program brought about by the COVID-19 pandemic. According to MC 009, s. 2020 entitled Omnibus Guidelines in the Implementation of the Emergency Subsidy Program of the Department of Social Welfare and Development and is available at https://www.dswd.gov.ph/issuances/MCs/MC_2020-009.pdf, it is the barangay that chooses the SAP beneficiaries. Specifically: X. Role of the LGUs The LGU shall perform the following: xxx 2. Prepare the list of target beneficiaries/clients as stated above; xxx Thus, you could inquire at your barangay regarding your non-inclusion and possible addition as well as procuring a SAP form and a barcode because forms are printed by the LGU.
