The work site where the employees concerned are assigned is at the PNB Makati Bldg., 6754 Ayala Avenue, Makati City.

Thank you for your request dated Aug 02, 2022 12:52:30 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.
You asked for [1] DOLE WSFC Report Form, [2] BWC-WHSD Form4 & [3] RKS Form 5 of 2020.
It is with deep regret that we could not grant your request as it falls on one of the exemptions provided in the FOI.
"Information, documents or records known by reason of official capacity and are deemed as confidential, including those submitted or disclosed by entities to government agencies, tribunals, boards, or officers, in relation to the performance of their functions, or to inquiries or investigation conducted by them in the exercise of their administrative, regulatory, or quasi-judicial powers;" is considered as part of information restricted for access of the general public.
Unless you are party to the case, any concerns on the rightful grant of benefits may be resolve by raising the concern on the DOLE RO/FO having jurisdiction to the concerned workplace.

In relation to my request per Tracking no: #DOLE-004004026114, what does this response actually mean?
"xxx Unless you are party to the case, any concerns on the rightful grant of benefits may be resolve by raising the concern on the DOLE RO/FO having jurisdiction to the concerned workplace. xxx"
I am the complainant in the labor case already filed before the NLRC for which the requested information and documents will be needed for the proper disposition by the labor arbiter.
I can provide the docket number which you can doublecheck in the NLRC website. Please let me know the process.
