Hi! I am John Vincent Doctama, a graduating BS Business Economics student from University of the Philippines Diliman, School of Economics. I am currently doing my undergraduate thesis. With this, may I request for anonymized socioeconomic data from application forms for medical social service grants for the years 2018 to 2023, specifically for children who experienced unintentional injuries, including those who were treated AND not treated for the years 2018 to 2023. Here are the specific information I would like to request: 1.) Child's Information a. Type of injury suffered b. Age c. Sex d. Education level e. Health insurance dependent (Yes or No) 2.) Parental Information a. Age of Father b. Age of Mother c. Highest educational attainment of father e. Highest educational attainment of mother f. Occupation of father g. Occupation of mother h. Marital status of parents 3.) Household information a. Number of working individuals in the family b. Combined family income c. Family type (e.g., single, extended, or non-related members) d. Number of family members e. Type of housing (e.g., informal settler, rented, owned, or staying with relatives) f. City of residence. Thank you so much

Thank you for your request made under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

We have received your request on Data on Medical Social Service Grant Applications (2018-2023) for children who experienced unintentional injuries. Kindly take note of the following:

Prior to our endorsement of your request to the concerned department,  there are certain procedures being implemented in PGH that you or your team need to undertake for research purposes.   Since PGH is under UP Manila, research protocols and data gathering procedures that are currently implemented in UP Manila are also being observed in PGH.  

We would like to first recommend seeking endorsement from the UP Manila Research Ethics Board.  It can be accessed online through the UPMREB website (reb.upm.edu.ph).  Subsequently, you need to register to the Research Grants Administration Office (RGAO) via online. Once appropriate clearances have been obtained, the PGH Expanded Hospital Research Office (EHRO) will issue a permit for the request to proceed.  

For clarification and other inquiries pertaining to the conduct of research in PGH, the EHRO can be reached at telephone number 8554-8400 local 2064.  Its office is located on the second floor of UP PGH.
