For Vector Files Request (in *.shp/ Web Map Service (WMS)) - you may go to goo.gl/PRTYxR [choose “Vector/*.shp” in the Data Format] or https://gisweb.phivolcs.dost.gov.ph/gisweb/geomatics-services-request to start the process of your request. However, please note that we will only provide vector data once we receive a signed Data User Agreement form (for LGU, Academe and Other Gov't Agencies) or a Memorandum of Agreement form (for NGO, Private Company/ Personal) use. The form will be provided upon submission of requests through the link. For other details related to this, please send an email addressed to Teresito C. Your right to request a review
If you are dissatisfied with our response to your FOI request, you have the option to request an internal review. To initiate this process, kindly submit your written appeal for review addressed to DOST Assistant Secretary for Administration and Legal Affairs, Dr. Diana L. Ignacio, DOST Complex, Gen. Santos Avenue, Taguig City with email [email protected]. Please provide a detailed explanation of your dissatisfaction with our response within 15 calendar days from the date of receiving this response. We will conduct the review and communicate the outcome within 30 calendar days from the receipt of your review request.
If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
