While our primary objective is to provide information whenever possible, we regret to inform you that NTC is not the repository of all registered SIM Card numbers and we do not have any access to it. However, we recommend that you visit the concerned TELCO nearest you and seek for further assistance regarding that matter.
Please refer to Section 6 of REPUBLIC ACT NO. 11934, October 10, 2022 otherwise known as SIM Registration Act (SRA):
Section 6. SIM Register. All PTEs shall maintain their own database containing information required under this Act. The database shall strictly serve as a SIM Register to be used by PTEs to process, activate or deactivate a SIM or subscription and shall not be used for any other purpose, unless otherwise provided under this Act. The successful submission and acceptance of the required registration form shall serve as the certification of registration by the end-user.
The registration required under this Act shall be implemented at no cost to the end-users.
In the recordkeeping of information, PTEs shall ensure that the end-users' data are secured and protected at all time. The PTEs shall comply with the minimum information security standards prescribed by the DICT consistent with internationally accepted cybersecurity standards and relevant laws, rules and regulations.
The DICT shall establish and perform an annual audit on PTEs' compliance with information security standards.

4. For the request of blocking of lost cellphone unit,
Upload the following required files:
A) Copy of Valid Identification - Any government-issued ID - For students, School ID. For cases when ID is not available, Birth Certificate OR NBI Clearance
B) Affidavit of Loss and undertaking form;

regional office by searching their addresses at www.ntc.gov.ph under the
menu Regional Office.
5. For complaints and concerns related to SIM registration, you may contact NTC's 24/7 consumer hotline 1682 or DICT's complaint center hotline 1326 for that matter;
6. For concerns regarding replacement of your registered SIM Number, you report personally to the respective TELCO Provider;
7. For Smart subscribers, you may visit their official domain: https://smart.com.ph. Subscribers are encouraged to report suspicious messages via its HuliScam portal at https://smart.com.ph/HuliScam. Subscribers can provide the following details:
a. the sender’s mobile number or alphanumeric identifier,
b. the message content,
c. the date received,
d. recipient’s location.
Fraudulent SIM numbers are promptly blocked following verification.
