Hi, I would like to request for the cancellation of my PWD ID (expired) so I can renew it here in the Municipality of Plaridel, Bulacan. Apparently, my PWD ID number was not found in the database of DOH. I was told that I need to go to the office where I got my ID. However, that is in Manila. So, I was hoping if this can be canceled here. Here are my details: KARL JAY HENRI CHUA MALE PWD ID NO: 13392869336 VISUAL DISABILITY (TOTALLY BLIND - RIGHT EYE) DOB: 11/21/1989 DATE OF ISSUE: 04/06/2019 Thank you for looking into this.
