To whom it may concern, I am seeking information for the list of requirements needed to claim death benefits of a SEA-BASED OFW (My Father) who died in Germany, in the ship, due to heart attack, last May 2024. We are informed that claiming the funeral and death benefits with ECC is through SSS. Tried seeking information with SSS but they are requiring too many documents and cannot provide a clear instructions on how to file for the said death benefits with ECC. We only have the papers from Consular office. We currently have the following: Report of Death Pre Medical Certificate Post Medical Certificate from the German Authorities And other personal documents of the OFW. Can you provide us a clear step by step procedure on how to file and a detailed list of requirements needed. Thank you so much. Looking forward for a response. Regards, Patricia Aivy Joson

Please note that if your father has a surviving legal spouse who is also a member of the SSS, she herself can initiate the filing via My.SSS portal, otherwise, filing is administered in person with the SSS nearest to you.

Your right to request a review

To initiate this process, kindly submit your written appeal for review addressed to [insert name of Secretary and postal/e-mail address]. Please provide a detailed explanation of your dissatisfaction with our response within 15 calendar days from the date of receiving this response. We will conduct the review and communicate the outcome within 30 calendar days from the receipt of your review request.

Should you remain unsatisfied with the review outcome, you retain the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
