This refers to your request received under Executive Order No. 2 (s. 2016) on Freedom of Information (FOI) in the Executive Branch regarding your unemployment benefit request.

To address your concern, kindly submit an application with the Social Security System (SSS) branch nearest you to secure a transaction code before you can apply for a Certificate of Involuntary Separation with our Office. For local workers, you may file your application for the issuance of Certificate of Involuntary Separation through DOLE Field/Provincial Office (for local workers) where the company or employer is located.
