We are pleased to inform you that your FOI request has been approved. Please be informed that you can access your Vaxcert by Downloading the eGov PH Application. Sign up in 3 simple steps:
1. Download the App - Install the eGov App on your smartphone or tablet through the App Store or Google Play Store.
2. Register an Account -Provide your mobile number and authenticate with a One-Time Pin
3. Verify your Identity - Enter your basic information and complete the facial recognition technology Once registered, you may Log in and select the service : HEALTH and choose VaxCertPH If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LOCAL GOVERNMENT UNIT (LGU) where you received your vaccination and request for assistance to generate your vaccination certificate.

Your right to request a review
If you are dissatisfied with our response to your FOl request, you have the option to request an internal review. To initiate this process, kindly submit your written appeal for review addressed to the Central Appeals and Review Committee (CARC) at [email protected] provide a detailed explanation of your dissatisfaction with our response within 15 calendar days from the date of receiving this response. We will conduct the review and communicate the outcome within 30 calendar days from the receipt of your review request. Should you remain unsatisfied with the review outcome, you retain the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
