Hi i would like to request to department of foreign affairs to allow me to get a replacement of may loss passport i have a passport using the surname of my husband MAURINE ACAIN RODANO but sadly my passport was not given back to me when im in qatar and return here in Philippines together with the travel documents. So i get new one but my mistake was i used my maiden name MAURINE IDUL ACAIN but its not intentionally to use it and sadly loss it last october so now im applying for replacement of my passport and i want to use the surname of my husband MAURINE ACAIN RODANO i wish and request to grant it ..
