If you plan to personally have your CAV processed, you may have it done at the following locations, depending on where you graduated:
For those who graduated outside NCR, you have two options:
1) For those who graduated outside NCR, you may process CAV at the CHED Regional Office of the region in which your school falls under; you may find the list of regional offices here: https://ched.gov.ph/regional-offices/
2) For those who graduated outside NCR, you may process CAV at the Office of Student Development and Services (OSDS) at the CHED Central Office located at Higher Education Development Center Building, C.P. Garcia Ave, Diliman, Quezon City.
For those who graduated from a school within NCR, you must go to the CHED Regional Office for NCR and NOT the Central Office. They are found at the Metalcasting Bldg., Quezon City University, 673 Quirino Highway, San Bartolome, Novaliches, Quezon City.
As for the requirements, if you will be applying for your CAV at the CHED Central Office, you will need a certified true copy of your TOR and a certified true copy of your diploma. If you will process your CAV through the CHED Regional Office mentioned above, an additional requirement of an endorsement letter from the school for CAV is required. Please be informed that records such as the TOR and Diploma are to be requested directly from the school as CHED does not have a copy of these documents unless the school has closed down.
There is a processing fee of Php 80 per set and, as per our Citizen's Charter, processing will take 7 working days. The duration of the processing period may also depend on the number of citizens applying for CAV and can make the processing period of 7 working days longer.
