You asked for Tax Exemption Certificate for OFW.
Response to Request
This is in connection with your request for information regarding Requirements for Japan Visa Application. The Checklist of Documentary Requirements are as follows:
1. Certification of Low Income/No Income signed by the Barangay Chairman of the place where the applicant resides;
2. Duly Notarized Affidavit of Low Income/No Income from the Notary Public;
3. Request Letter;
4. Photocopy of Identification Card with three (3) specimen signature;
5. Payment of Certification Fee (P100.00) and Loose Documentary Stamp Tax (P30.00), per copy;
6. If transacting through a representative, a Special Power of Attorney (SPA) shall be executed by the taxpayer-applicant and Any government-issued IDs of the Taxpayer and Authorized Representative.
Submit the above mentioned documents/requirements at the Collection Section of the Revenue District Office (RDO) having jurisdiction over the place of Residence of the Taxpayer.
