Thank you for your request dated Sept. 27, 2024, made under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.
In reply, please be informed that we called your mobile number today (November 4, 2024) to confirmed your request on "Personal Data". We were apprised that you have been given a Certificate of Business Closure by Revenue District Office (RDO) No. 36-Puerto Princesa last December 6, 2023 and February 14, 2024.
You have been advised to accomplish and send (via email) your duly accomplished BIR Form 1905 [https://bir-cdn.bir.gov.ph/BIR/pdf/1905%20Jul%202021%20ENCS%20-%20Final.pdf], Certificate of Business Closure and your two (2) valid government issued I.D.s to [email protected] to reflect the necessary change in your current taxpayer type.
As to your other concern on requesting for a copies of your business closure documents, we have coordinated your request to RDO 36-Puerto Princesa for necessary action, the aforesaid documents cannot be uploaded in the FOI Portal because if contains sensitive information. A member of the Client Support Staff/Team will be contacting your mobile number regarding your request for a copies of your business closure documents.
Any request for certification that may be requested by a taxpayer from the RDO on matters relating to its registration shall be charged with a fee in an amount not exceeding one hundred pesos (P100.00), in addition to the documentary stamp tax of Thirty Pesos (P30.00) imposed under Section 188 of the National Internal Revenue Code, as amended.
Your right to request a review

Please provide a detailed explanation of your dissatisfaction with our response within 15 calendar days from the date of receiving this response.
