We wish to inform you that your BIR Form 1701 for the year 2023 is available and ready for release provided that you submit/present the following requirements:
- Letter request indicating the purpose with signature;
- Present and photocopy any Valid Government ID;
- Proof of Payment of Certification Fee Php 100.00 using the BIR Form 0605 (MC);
- Documentary Stamp Tax (Php 30.00);
- Affidavit of loss for the lost return;
- Proof of payment for ITR’s tax due;
- Special Power of Attorney if transacting through a representative
