Hi, I'd like to ask for the list of requirements & process for correcting an R.A. 9225 Identification Certificate in relation to Section 14 of BI Memorandum Circular No. AFF-04-01. Thank you.

Attached is the checklist of requirements for the issuance of CRPC(Identification Certificate), note that the second bullet point requires the submission of a letter request. For correction of RA 9225 Identification Certificate you may indicate it on the said letter request and submit it together with the rest of requirements on the checklist.
