Hello, Good day! My name is Leonora Paulina Catriz Simbulan. I am a graduate of the ALS back in 2011 at Pasay City East Highschool. I would like to request for a Certified Original Copy of my ALS Diploma,Form-137-138 & Good Moral Certificate as I have misplaced my original copy. I am requesting for another original one because I am applying for Tesda this coming school year. I would be willing to pay any extra charges for this. Hope you can get back to me as soon as possible. Looking forward to hear from you. Thank you!

Thank you for your request dated May 04, 2020 03:42:57 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Request for Diploma,Form-137-138 & Good Moral Certificate. Response to Request Please be informed that the Department of Education (DepEd) Central Office (CO) does not have copy of learners’ school records. Hence, you are advised to get certification from the school where you graduated and bring it to the Schools Division Office (SDO) for a certified copy, since original school records are issued once only. If you need this for your job application abroad, you will bring the certification or school records to the Regional Office (RO) for certification, authentication, and verification (CAV) purposes. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request. If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011). Thank you.
