Hello,
I am a former student of St Peter Christian school in Holy Spirit, Quezon City which I understand has closed 13 years ago. Upon completing my studies, I owed a balance that was never paid before I came back the the U.S (where I was born) and never received my diploma. I am in desperate need of assistance as I graduated in 1994. I have not had the need for this document as I have been a business owner for this time. You may refer to the Guidelines on Learner's School Records in the DepEd Order No. 54, s. 2016 titled "Guidelines on the Request and Transfer of Learner’s School Records" regarding your request for a Form-137 (Permanent Record), Form-138 (Report Card), and other school records for your information and guidance:
