Hi! I would like to request a copy of my Grade 9 Form 138 (Report Card) in A.Y 2018-2019 or an issuance of lost/missing Form 138 on or before applications for Universities start later this year for A.Y 2023-2024. I am currently in Grade 11 (A.Y. 2021-2022) and I have been preparing the minimum requirements for the past weeks. The case is, most of the uni I have inquired to asks for Grades 8-11 report cards and my Grade 9 is not with me. I have my Garde 9 School ID (A.Y. 2018-2019), Pre-recognition Certificate, and NCAE result for follow up verification. Thank you!

Please be informed that the Department of Education-Central Office (DepEd-CO) does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you need to bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017).
