Hi I would like to request to change my surname to my vaccine card but still same information. The declared record of mine was Rym Herchel Alpajora and I want to make a request to change my name to Rym Herchel Custodio to make it similar to my passport records, because I need this to work abroad. Custodio was given since birth and I used Alpajora on my wedding day with my Affidavit of Discrepancy for my supporting document.

Your FOI request is approved.

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. The vaccination record may
have been incorrectly encoded in the Vaccination
Information Management System (VIMS) by the
vaccination center.
Kindly coordinate with the LGU where you
received your vaccination and request for
assistance to correct your vaccination record and
certificate.
