Hi, I lost my vaccine card and forgot when did i got my first dose and second dose. I tried to request a vax cert but it say no record found. I was vaccinated around August 2021 to october at baguio city. My booster was janssen vaccine last april 28, 2022 at san fernando city la union

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
