Good Day, 

I would like to ask  a request on Department of health (DOH) to update my vaccine record, I am done on filling up online but Ilmy record not found. I want to get a Vaxcert certificate that I will be using  for applying a Job. Vaxcert is one of the requirements of the company where i apply. I hope it will process it directly because I have a deadline of my requirements on  September 19, 2023.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
