Dear FOI,

I  would like to have my Vaxcert updated in the system as my second booster is not recorded. As I am about to start my placement in a disability facility here in Australia, it is crucial that my vaccination certificate is up-to-date and reflected accurately in the system as soon as possible. 

Below is my vaccination details:

First dose (SINOVAC)  06/16/ 2021 Quezon City
Second dose(SINOVAC)  07/14/ 2021 Quezon City
First booster (MODERNA) 01/29/2022 Quezon City

MISSING:
Second booster (MODERNA) 07/25/2022 Makati City

You asked for Vaxcert missing dose.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
